SURVEY & RESEARCH TEMPLATES
Start with a question you can actually use.
Choose a questionnaire, a decision brief or a results note. Edit the fields, keep the answer choices with each question and download the draft for your study.
Show the same checkout task to each person. Ask about the task they just attempted. Compare the price they report with the actual total; an ease rating alone cannot establish comprehension.
See the complete before-and-after repairQuestionnaires use fictional situations. Planning and results fields are prompts to complete. Edits stay in this page session; copy or download to keep them.
Before you send your questionnaire
Replace the fictional situation with the task your respondents will see. Check whether every answer choice fits the question, including a way to say they cannot judge when that is a reasonable response. Keep coding instructions out of the respondent-facing survey.
Use “Review these questions” to load your edited question field into the checker on this page. It applies wording rules locally; it does not validate a scale or interpret the study plan. Read the draft with people from your intended audience before fieldwork.
Your edits stay in this page session. They are not saved by the site or included in the URL. Copy or download what you need before leaving or reloading.
